For quick reference, below is a summary of benefits offered by the District. Important Benefit Notices
1095-C Will Be Available in THEbenefitsHUB By March 2, 2026
Your form 1095-C Employer-Provided Health Insurance Offer and Coverage will be available to download from THEbenfitsHUB by March 2, 2026. The Form 1095-C contains important information about the healthcare coverage offered or provided to you by the District. Information from the form may be referenced when filing your tax return. Please see below for some frequently asked questions.
1) When will the District provide my Form 1095-C?
2025 forms will be available in THEbenefitsHUB no later than March 2, 2026. The District no longer mails the Form 1095-C. Please see the below notice for further information.

3) Why do I need the form 1095-C?
Your 1095-C will be referenced when you complete your taxes. The form itself is not sent to the IRS, it remains with your tax documents. However, your tax preparer may require it in hand to complete your taxes. You may think of the form as "proof of insurance" for the IRS should they request it, so you should keep it with your tax forms.
4) How many 1095-C forms will I receive?
You may receive form 1095-C for any company you worked for during the tax year. However, you should only receive one 1095-C per employee from the District. If both spouses are employed by the District, you may receive two forms.
5) What do all of these codes mean?
A detailed explanation of each code can be found on page 2 of the form (click the link above). However, each of the below lines have a different significance:
- Part 2, Line 14 is used to report whether an offer of coverage was made to an employee for each month of the year. Please note, this does not indicate you elected, or paid for, medical coverage; it only indicates if the District offered it.
- Part 2, Line 15 is used to report your share of the lowest-cost monthly premium for self-only qualifying coverage, whether you elected coverage or not. The amount reported on line 15 will be present even if you did not elect coverage, and it may not be the amount you paid for coverage. It is only the lowest option available.
- Part 2, Line 16 explains why your employer did or did not offer you coverage.
- Part 3 shows who accepted the offer and is actually covered.The box will be checked for any month that you were enrolled in coverage for at least one day.

They answer 24/7 and can help people in over 150 languages. 211 is available throughout the entire state of Texas and 95% of the United States. They have resources for housing, food, employment, clothing, rent and mortgage assistance, utility bill assistance, legal services, veteran and military member needs, mental health, health care, and so much more.

After Enrollment - What to Expect
What to Expect After Enrolling (Annual and New Hire)
Annual Enrollment
- Effective Date: 9/1.
- Medical deductible and annual maximums start of 9/1;
- Dental deductibles and annual maximums start 1/1;
- Vision deductibles and annual maximums renew 12 months after your last date of service.
- Cards: You will receive new cards only if you changed plans or newly enrolled. Cards are expected by the second week of September. You will receive separate medical, pharmacy, dental, and vision cards.
New Hires
- Effective Date: Medical the 1st of the month after your hire date; all other benefits the 1st of the month after 30 days from your hire date. In many cases this will be the same date this year, 9/1.
- Cards: You should receive cards within 3 weeks of completing your enrollment. You will receive separate medical, pharmacy, dental, and vision cards.
- 13th Check: voluntary benefit elections will not be deducted from the 13th check. TRS, regular taxes, voluntary retirement contributions, and other deductions such as garnishments will be deducted. If you elected medical effective on your hire date, the missed deduction from August will be taken on your September check.
ALL STAFF: PLEASE REVIEW YOUR SEPTEMBER AND OCTOBER PAYCHECK(S) AND MAKE SURE DEDUCTIONS ARE CORRECT. ERRORS FOUND MORE THAN 30 DAYS AFTER THE EFFECTIVE DATE MAY NOT BE CORRECTABLE.

District Paid Telehealth - Amazon One Medical
District paid telehealth service is at no cost to you or your covered dependents (spouse and children under 26) as long as you have listed them in THEbenefitsHUB. You do not have to be enrolled in medical to access Amazon One Medical.
- Visit onemedical.com/mybenefit, scroll down and select Activate Sponsored Membership;
- Use your email for registration. If your personal and birdville email doesn't work, or you are unsure which email is listed, use code BIRDXOM.
- Once registered, use the Urgent Video Chat feature for no-cost telehealth services. Do not click Remote Visit, this will be charged to your insurance or bill you privately.

Blue Cross Blue Shield Medical

What's Changing for 2025-2026
Only Medical rates and the telehealth provider are changing. Below is a quick recap of important points:
Medical - Medical rates are increasing by 9%, but only minor plan changes are being made. The District contribution increased for employees in certain job classes as stated on the rate sheet. If you change plans, you will receive a new card. Otherwise, you will not receive a new card unless you request one by calling. For questions about TRS ActiveCare plans and assistance, please call 1-866-355-5999.

District Paid Telehealth Provider - The District paid telehealth provider is changing from MDLive to Amazon One Medical. Amazon One Medical provides a user-friendly platform and service nation-wide for you and your immediate family.
You will be required to create a new account with Amazon One after 9/1/25.
Amazon One Medical Registration Instructions - COMING SOON
MDLive will no longer be paid for by the District. However, it is still available through your TRS medical plan with the applicable co-pay or co-insurance of your plan.
Dependent Social Security Number requirements - TRS and the Affordability Care Act (ACA) now require dependent social security numbers. Please ensure you enter all dependent socials in THEbenefitsHUB. If you choose not to list your dependents in THEbenefitsHUB, they will not be covered by the District paid telehealth.
Life Insurance and AD&D - No change unless you are moving to a new age band; if so, you may have a premium increase.
HSA - HSA limits will increase to $4,300 individual coverage and $8,550 family coverage. The catch-up contribution (55 and older) limits remain at $1,000. NOTE: Your household may not be enrolled in both the FSA and HSA during the same plan year.
FSA and DCA- Health Flexible Spending limits will increase to $3,300; Dependent Care limits will remain at $5,000. You must re-enroll in these benefits every year, they do not carry from year to year. NOTE: Your household may not be enrolled in both the FSA and HSA during the same plan year.
457 and 403(b) - Contribution limits are currently $23,500 each with a catch-up contribution (50 and older) as follows:
age 50-59 $7,500
age 60-63 $11,250
age 64 and over $7,500

How to Enroll:
Step 1: View the 2025-2026 Enrollment Guide or links above for rates and benefits available;
Step 3: If you are newly enrolling in the ActiveCare Primary or Primary+ plan, check if your medical providers are in-network at the BCBS Provider Search Tool;
Step 4: If you are new enrolling in a dental plan, check if your dental providers are in-network for the PPO and the HMO dental; It is highly recommended to call the provider to confirm they are in-network, especially for the HMO dental. Provider networks change frequently and the website may not reflect the changes.
Step 5: Beginning 7/21/2024, enroll by going to THEbenefitsHUB enrollment site, http://www.mybenefitshub.com/birdvilleisd then click Log In in the top right corner. Helpful information will be available at the top of each benefit page as you go through enrollment.

Benefits While on Leave
If you miss a check, you will be required to submit payment for any benefits normally deducted from your paycheck. You will be notified via email by the benefits office of any missed deductions. You must submit payment within 30 days of the missed deduction or your benefits may be cancelled effective the first of the month in which full premiums were not paid.
If you are being paid through the disability provider (CHUBB) or through Worker's Compensation and are not receiving a check through payroll, benefit premiums are NOT taken from those payments. You will be required to submit benefit payments to the district in either of these cases.
As long as you are on FMLA and/or are receiving a paycheck, the district's contribution to medical coverage will apply. However, if you exhaust FMLA and are no longer receiving a paycheck, you will be responsible for payment of your full medical premium.

Claim Filing
You will receive an email from the Benefits Office regarding payment of benefits and a list of benefits you are enrolled in and may need to file a claim for. Claim filing instructions and forms are listed below.
- Disability Claims (CHUBB) - Group ID is 100000218. You may file a claim by telephone or by logging into the Chubb portal. Instructions for both are below. If you use the telephonic submission, it is still recommended to create your account on the portal to easily receive updates, upload ongoing documents, etc.

Sick Leave Bank
Sick Leave Bank provides additional sick leave days to members of the Bank who are unable to perform the duties of his/her position in the event of their own or an immediate family member's critical or extended illness or disability. When it becomes apparent that a member will exhaust all accumulated state and local leave and vacation days, then and only then may a request be initiated.
- Bank days cannot be used for maternity leave, but may be requested for complications arising from pregnancy;
- Bank days cannot be used for Worker's Compensation leave;
- The employee must miss five (5) consecutive days; All leave days, including vacation days, must be exhausted;

To verify if you are in the BISD Sick Bank, log into THEbenefitsHUB. Click log-in in the upper right corner. Once in the THEbenefitsHUB, go to Benefit Plan Information, then Benefit Plan Enrollment to see if you are enrolled.
Leaves and Paycheck Calculation
If you do not have enough leave days to cover your entire leave, you will be “docked” the days you are scheduled to work. Everyone’s situation and calculation is different, but the basic calculation is below:
What you need:
- Your regular salary (annual salary divided by 12) and daily rate (annual divided by days in contract). Both can be found on your pay stub and are listed under rate and current.
- The payroll schedule, 25-26 Semi-Monthly 25-26 Monthly
- Count how many actual work days you are not working each pay period. Keep in mind days you are not scheduled to work (like spring break) don’t count as missed days.
- Calculation Worksheet
Examples are provided below. Please note, examples are only intended as high-level calculations and do not include all scenarios. Your results may be different depending on taxes and other variables.
Monthly Sample
Pay period (Monthly) salary $5000 15 X 320.85 = $4812.75
Daily rate $320.85 $5000 - $4812.75 = $187.25
Miss 15 days from pay period Gross pay (before taxes and deductions) $187.25
It is possible in this scenario you will still owe benefit premiums.
Bi-weekly Sample

Miss 8 days from pay period Gross pay (before taxes and deductions) $209.68
It is possible in this scenario you will still owe benefit premiums.
- If your gross pay is positive, this is money owed to you.
- If your gross pay is negative, this is money you owe the district.
- Negative balances will be carried over to the next pay period until paid in full.
- If you separate from the district with a negative balance (overpaid status), you will be required to repay the district.
New Hires and Eligibility
NEW HIRES AND ELIGIBILITY
Employees in positions regularly scheduled to work 20 hours or more per week are eligible for the full range of benefits offered by Birdville ISD. Employees regularly scheduled to work less than 20 hours per week are eligible to enroll in medical coverage but do not receive the district’s contribution towards medical. New Hire Enrollment Information and Guides
Your enrollment deadline is 30 days from your first contract start date. Not sure of your start (contract) date? A cheat sheet for contract dates can be found here. If you have any problems making this deadline, please contact the benefits office immediately. You will not have another opportunity to enroll until open enrollment effective 9/1 the following year.

- Please note, the District provides different contributions to medical depending on your job class. Examples of each job class are provided on the rate sheet.
2) Enroll by clicking here and logging in to THEbenefitsHUB (right top corner)
- All employees must log in and either enroll/waive benefits, the process is the same. Please go through the whole process and continue until you can't continue anymore. Even if you don't want additional benefits, the district pays for Basic Life insurance and Telehealth for you and enrolled dependents. These are free, but you must list your dependents and enter beneficiaries for the life insurance.
IMPORTANT NOTE: If you are a new hire in the months of July or August, please do not log into THEbenefitsHUB before July 21. Not all benefit options will be available to you until after annual enrollment begins for all other employees. July hires will be required to complete both new hire enrollment (effective 8/1) as well as annual enrollment (effective 9/1), in two separate steps. Keep going until you receive the congratulations message; if there is a continue button at the bottom of the page, you are not done!

Health Care Related Enrollment
Employees new to the district or employees moving from a part time to full-time benefit eligible position must enroll in or waive benefits in the first 30 days from their actively-at-work date (start date). Medical is effective the first of the month after your hire date, but may be made effective on the employee’s hire date at the employee’s written request. All other benefits, including district paid benefits, are effective on the first of the month after 30 days of employment.
If moving from one TRS ActiveCare district to another, you will need to elect the same medical plan from your previous district (unless declining). TRS does not allow mid-year plan changes, even if you are changing districts. In addition, TRS does not allow dual coverage, please make sure your medical with your prior district ends before your benefits are effective with BISD. In addition, if you are changing districts benefit enrollment does not roll over to BISD.
Benefit elections must be made by going to the District’s Benefits Portal and Login in the top right corner. Once enrollment is complete, employees have access to the HUB (either online or on a mobile device) to review their benefit elections and change beneficiary information. Please remember, once you have made your new hire elections and have passed your first 30 days, you will only be able to make changes during Annual Open Enrollment or due to a qualifying event mid-year. The HSA is the only exception to this rule. More information about these changes follows.
The District deducts premiums for the month benefits are effective, not in advance. For example, benefits active in October are deducted in October. It is imperative you confirm your changes take effect on your check and notify the Benefits Office of any discrepancies in a timely manner.
Sick Leave Bank Enrollment
Employees who will be granted at least 3 days of local sick leave upon hire will be eligible to enroll in the District's Sick Leave Bank. The Sick Leave Bank can be used for catastrophic illness for you or your immediate family if you have exhausted all of your days (accruals). Enrollees donate just 3 local days once – you do not donate 3 days each year, and there is no monthly premium. For more information, view the benefits portal here. If you choose not to enroll, or are not yet eligible at the time of hire, your next opportunity to enroll will be during Annual Enrollment for a September 1 effective date.
Teachers Retirement System (TRS) Enrollment
The District automatically enrolls every eligible employee in TRS. TRS is a pension system, and takes the place of Social Security. Birdville ISD earnings are not reported as Social Security earnings, but are reported as Medicare earnings. Both the District and employee contribute to the TRS pension program. The employee contribution effective 9/1/2023 is 8.25%. You are also required to contribute to TRS Care, which is similar to a Medicare tax, but goes towards TRS retiree health. The TRSCare employee contribution rate is .65%. You may not opt out of either of these contributions.
You are strongly encouraged to register with the TRS website, MyTRS. Once registered, you can view your contributions, change your beneficiaries, estimate your retirement, and much more. If you are new to the TRS system, you will receive a welcome letter with your member ID 1-2 months after your employment begins. Once you receive this letter, you may register for MyTRS.
403(b) and 457 Enrollment
You may enroll in the 457 and 403(b) plans at any time, but each has a different process. You will have the opportunity to enroll in the 457 during your new hire enrollment process on THEbenefitsHUB, or anytime after. The 403(b) must be set up by a vendor of your choice who will notify the District. Click here for a quick reference on these retirement plan options. Wellness and Mental Health
WELLNESS AND MENTAL HEALTH SERVICES
Birdville ISD provides a number of wellness initiatives throughout the year geared toward improving various areas of wellness including onsite vaccinations, stress and time management, health coaching, and more! Information about the district’s current wellness initiative can be found below. All employees are eligible to participate in the wellness initiatives, regardless of part or full-time status, or enrollment in other benefits.
Wellness Campaigns
The Spring 2025 Wellness Campaign, Thrive, is here with a journal for anyone who registers! Registration must be received by Friday, February 13th to ensure your prize. Birdville's campaign begins February 9th, when you should begin receiving weekly challenge emails. You may enroll using the online registration link, or submit a paper sign-up sheet to Susan Dippolito via interoffice mail or Email Susan.

Mental Health and Counseling Services
Both the district Employee Assistance Program (EAP) and TRS Active Care provide access to mental health and counseling services. Don't forget the EAP is free to use for up to 5 face to face visits. More information can be found at the websites below:

The Cancer and Critical Illness plans offer a wellness benefit of $50 per year. If you are enrolled in one or both of these benefits, simply submit a wellness claim after having your annual screening(s). Claim information can be found below.
District Provided Services and TRS ActiveCare Walk-In Clinics
District provided - The district may offer a number of on-site clinics including flu vaccines and B-12/lipotropic shots, dental cleanings, etc. throughout the year. As the schedule for clinics becomes available, it will be emailed as well as posted on this page.
TRS ActiveCare provided - There are a number of walk in clinics available for vaccines and to treat minor illnesses, injuries, physicals, screenings and monitoring for chronic conditions. To find a walk in clinic for your plan, use the following:

Retirement and Planning
RETIREMENT
Retirement may feel far away, but it’s never too early to start planning. Birdville ISD offers several retirement savings options, including the Teachers Retirement System (TRS) pension. Understanding how TRS, Social Security, and other savings plans work together is important.
TRS Pension Benefits
All regular District employees who work at least four and one-half months are members of the Teacher Retirement System of Texas (TRS). As of September 1, 2023, employees contribute 8.25% of eligible pay to the pension and 0.65% to TRS-Care (retiree health insurance). Your retirement formula is based on your TRS start date.
TRS also offers helpful videos and the option to schedule a private virtual appointment with a TRS benefit counselor. Additional resources are available below.

Voluntary 457 and 403(b) Retirement Savings Plans
Birdville ISD offers two voluntary supplemental retirement plans: 457 and a 403(b). These plans allow you to save additional retirement funds on a pre-tax or post-tax. The District does not contribute to these plans. You may enroll in the 457 or 403(b) at any time, though each plan has a different enrollment process. IMPORTANT NOTE: You may receive retirement-related emails from financial providers. TCG is the only investment vendor contracted by the District and administers the 457 and 403(b) plans. TCG has a fiduciary responsibility to act in your best interest. Other vendors may be legitimate but are not vetted by the District and are not authorized to meet with employees on District property or use District resources to solicit employees. The District will only share financial readiness announcements and seminar information through the Communications and Marketing Department.

- Easily open and manage your account and contributions online or by using the TCG Mobile Retirement app.
- Enrollment and contribution changes take effect the 1st of the month after the request is made.
403(b) Retirement Savings
- Enroll any time by visiting www.region10rams.org (click Access my Plan) and review the 403(b) Approved Vendor List.
- Contact a vendor of your choice directly to open an account.
- Once you have opened the account, set up your contribution by contacting TCG Services at (800) 943-9179 or online at https://region10rams.org/2022/10/18/403b-enrollment-guide.
- Enrollment and contribution changes take effect the 1st of the month after the request is made.

Social Security
The District and its employees do not contribute to Social Security, so your Birdville ISD service and earnings will not appear on your Social Security statement. Employees do contribute 1.45% of eligible pay to Medicare.

Effective January 1, 2026, you may receive both Social Security and a TRS pension without penalty.

It’s recommended that you begin the retirement process about six months before your planned retirement date. The steps and resources below can help guide you.
1. Review the HR Website

2. Review Your Retirement Income Options
Consider all income sources, including Social Security, TRS, and any 403(b), 457, or other retirement plans. You may schedule a free virtual appointment with the District’s TCG representative, Kevin Hull, who can help you plan how to use your retirement resources and review rollover options. 3. Decide on Medical Coverage

4. Review Other Benefits
Decide if you want to continue dental, vision, or life insurance. If so, you must apply within 30 days of retirement through COBRA or a private provider. Click here for COBRA information.
